Home Visits

If a home visit is required, please telephone 01329 280887 - ideally between 8am - 10.30am. This helps us plan our visits. Whenever possible, if you can attend surgery you will be seen sooner and it will save the doctor valuable time.

Acute Visiting Service (AVS)

The Acute Visiting Service (AVS) is a service operating in Fareham to provide a quicker response and faster treatment for housebound patients with an acute illness.

Eight out of 10 GP practices are participating in the scheme including: Portchester Health Centre, Centre Practice, Gudge Heath Lane, Highlands Practice, Jubilee Surgery, Lockswood Surgery, Brook Lane Surgery,Whiteley Surgery.

The AVS team comprises of a Matron and 4 Locum GP’s.

The team will operate from 8am – 4pm Monday to Friday.

This service is for those patients who have an acute illness and require to be seen on the day.

Instead of having to wait until early afternoon for a GP from their own surgery to visit. We can provide an earlier visit to enable treatment if required to commence sooner or if they need to be referred into hospital, they are seen earlier which could be the difference between having to stay in overnight or not.

  • Quicker response to need
  • Improved service to patient
  • The ability to spend longer with each patient if required
  • Relieving the pressure on GP practices
  • GP’s can spend more time in surgery potentially creating more routine appointments.

The Acute Visiting Service began on the 10th September 2018 and in its first two months has dealt with over 100 Request for home visits.

If a housebound patient is in need of a home visit and they belong to one of the GP surgeries taking part in the service, they should contact their own surgery – please see overleaf for surgery contact details.

A receptionist will take all of the relevant details, speak to the duty Dr in practice and they will determine if the case is appropriate for one of the AVS team. The patient will then be contacted by the AVS team.
