To make an appointment with a Health Care Professional
To make an appointment with a GP or First Contact MSK Practitioner, please click on ‘Online Services’ and complete a Rapid Health triage questionnaire.
If you do not have an email address, or you do not have internet access (via smartphone, tablet or PC), then please phone or visit reception Monday-Friday between 08.00am-18.00pm and our reception team will be able to signposted you to the most appropriate healthcare professional, or alternative service.
The same day urgent triage list is managed by the duty team (the duty team is a GP and / or paramedic). There is no choice with who you speak to for an emergency appointment as this depends on who is allocated duty that day.
Routine face to face / telephone appointments can be booked up to 28 days ahead*. We offer appointments 08:00am to 18:00pm Monday to Friday and on Saturday mornings (the Saturday appointments are held at our Extended Access Hub which is located at Westlands Medical Centre, Westlands Grove, Portchester, PO16 9AD).
The majority of our appointments are ten minute duration. We ask that you try to observe the “One Problem per Appointment” ethos. We strive to deal with each medical problem to the highest standard and this proves to be impossible if we are asked to deal with a number of problems in one appointment slot.
We offer a number of clinics and services in the treatment room with our Nurses and HCA’s (Health Care Assistant) as well as appointments with our Social Prescriber, and Clinical Pharmacy team. These can be booked through the practice, and are telephone appointments (unless the Social Prescriber or Pharmacist arranges otherwise). Further information on our additional roles (social prescriber, pharmacy team, MSK etc.) can be found on our PCN webpage: https://farehamandportchesterpcn.gpweb.org.uk/our-primary-care-network
If you arrive more than 5 minutes late for your face to face appointment you may be required to rebook your appointment.
