ADHD Referral

Thank you for your interest in a referral for an ADHD assessment. This process is for adults or children aged 5 and above. For children under 5 please make an appointment through Rapid Health on our website.

To access an ADHD referral through Gudge Heath Lane Surgery you will need to follow this process.

STEP 1

  1. You can choose to access these assessments through the NHS pathway (PHL group for adults and CAMHS for children under 18).

For CAMHS a self referral can be made on the top right on their website by either school or parents (see link CAMHS) and you do not need to follow the remainder of the processes below.

If you are an adult over 18 and wish to use PHL group for your referral you need to continue to follow step 2.

  • If you do not wish to follow the NHS referral route you can have the option of going through the Right to Choose pathway which means you can choose to be referred to a private provider and this referral/assessment will be funded by the NHS. We cannot choose a private provider for you, and our staff cannot assist you in selecting or assessing the suitability of these providers for you. When choosing a provider consider

  1. Whether you meet their referral criteria
  2. That they can prescribe all medications indicated on a long term basis (please note Gudge Heath Lane Surgery does not prescribe any medications for ADHD from any providers)
  3. The waiting times and the type of assessment being provided

STEP 2

In order to process your referral you must complete all of the following

  1. A completed ADHD Screening questionnaire ADHD screening questionnaire . If you are looking for dual diagnosis assessment for Autism and ADHD you will need to also complete an AQ10 questionnaire.
  2. A letter explaining your symptoms and how these impact your day to day life both at home and work/ education. You will need to cover (where relevant) examples of inattention, hyperactivity and impulsivity along with the age you first noticed these symptoms. The more detail you put in this letter the less likely it will be to be rejected. This can be hand written or typed with your patient name, date of birth and address on it.
  3. Details of selected provider (right to choose or NHS)
  4. Any completed proforma that your chosen provider require you to complete- this can be found on each providers website. Without these they will not accept your referral.
  5. Visit the pod in the waiting room to undertake a blood pressure, heart rate and weight check.

 We can only accept your request for a referral for assessment once all of the above five pieces of documentation have been received.

 If we need to send you reminders for any outstanding information or documents, we will hold your referral for you for a period of 21 days, to allow you time to provide the required information. If you still do not respond to our reminders, we will assume you do not wish to be referred and we will close your request. Doing this allows our admin staff the time to help other patients.

STEP 3

Once we have received all five pieces of information, a member of our team will review your documentation.  This process can take up to 28 days. If you meet the criteria, the admin team will make the referral to your chosen provider, sending off all the required documents. After this, your chosen provider will be in touch.

If you don’t meet the criteria for referral, you will be informed and signposted to general support.

Once your referral has been sent to your chosen provider, the surgery will no longer be involved in the process and all queries must be directed to your provider.
